Three different numbers describe a compounded GLP-1 vial, and safety depends on never confusing them. Volume (mL) is how much liquid the vial holds. Concentration (mg/mL) is how much drug each milliliter carries. Total drug (mg) is volume × concentration. Your dose is prescribed in milligrams; your syringe measures volume (often in "units," where 100 units = 1 mL on a standard insulin syringe). The only safe bridge between them is the concentration printed on your vial's label — not a chart from a forum, not last month's vial, not a provider's generic instructions.

The worked example

Suppose a 2 mL vial at 5 mg/mL — 10 mg total. A 2.5 mg dose is 0.5 mL, or 50 units on a U-100 syringe. Now the refill arrives as a 1 mL vial at 10 mg/mL — also 10 mg total, same-looking box. The same 50-unit draw now delivers 5 mg — double the dose. Nothing was mislabeled; the concentration changed, and the volume habit didn't. What to confirm on every shipment

Then one calculation — your prescribed mg ÷ label mg/mL = mL, × 100 = units — written down before the first draw.
